  • Relationship between morphologic macular changes and visual outcome in eyes with persistent submacular fluid after surgery for macula-off rhegmatogenous retinal detachment and relevant factors of persistent submacular fluid

    Objective To investigate the relationship between morphologic macular changes and visual outcome in eyes with persistent submacular fluid (SMF) after surgery for macula-off rhegmatogenous retinal detachment (RRD) and the relevant factors of persistent submacular fluid. Methods Sixty-three consecutive patients (63 eyes) who underwent successful surgery for macula-off RRD were enrolled in this retrospective study. The patients were divided into three groups according to duration of SMF: no SMF group, short-term group (duration of SMF less than three months) and long-term group (duration of SMF more than three months). The follow-up ranged from six to 12 months. The morphologic macular changes, height of SMF and thickness of the outer nuclear layer (ONL) one month after surgery were assessed by spectral domain-optical coherence tomography (SD-OCT). The relationship between morphologic macular changes and logarithm of the minimum angle of resolution (logMAR) visual acuity and the relevant factors of persistent SMF were evaluated. Results Forty-five of 63 eyes (71.42%) presented morphologic changes after surgery. SMF was detected in 32 eyes (50.79%), which included 21 patients that had long-term course of SMF. Irregular thickness of the photoreceptor outer segments (OS) was observed in 23 eyes; disrupted inner segment/outer segment (IS/OS) junction was noted in 21 eyes, and disrupted external limiting membrane (ELM) was noted in 14 eyes. Irregular thickness of OS in long-term group was significantly higher than that in short-term group (chi;2=5.788, P=0.035). The proportion of IS/OS disruption and ELM discontinuation in long-term group was also higher than those in short-term group, but there was no statistically significant difference (chi;2=0.744, 0.375; P=0.472, 0.403). The postoperative visual acuity correlated positively with preoperative visual acuity and the time of retinal detachment (r=0.611, -0.374; P=0.007, 0.037). There was a significant difference of postoperative logMAR visual acuity (U=28.640, P=0.049) among no SMF group (0.27plusmn;0.26), short-term group (0.42plusmn;0.31) and long-term group (0.53plusmn;0.41). The postoperative visual acuity was insignificantly associated with irregular OS and IS/OS disruption (r=0.331, 0.320; P=0.073, 0.102). The irregular OS correlated positively with IS/OS disruption (r=0.388, P=0.027). The postoperative visual acuity in eyes with disrupted both IS/OS and ELM significantly decreased (U=29.920, P=0.036). The surgery manner (r=0.477, P<0.001), time of preoperative retinal detachment (r=0.354, P=0.047) and SMF height one month after surgery (r=0.375, P=0.039) were factors influencing persistent SMF. Conclusions The incidence of postoperative photoreceptor lesion was increased with time of persistent SMF. Disruption of both IS/OS and ELM after surgery means a poorer vision outcome. Duration of preoperative macular detachment, type of surgery and SMF heights one month after surgery were important factors for persistent SMF.

  • Analysis of associated factors and characteristics of different types of pathologic myopia maculopathy in fundus fluorescein angiography

    Objective To observe the characteristics of fundus fluorescein angiography (FFA) in different types of pathologic myopic maculopathy and evaluate the influence factor.Methods The clinical data of 251 patients (451 eyes) with pathologic myopic maculopathy were retrospectively analyzed. The patients were divided into 6 groups according to FFA characteristics: (1) lacquer cracks (LC); (2) choroidal neovascularization (CNV); (3) macular hemorrhage with LCs; (4) Fuchs spots; (5) macular atrophy; (6) macular hole. Their relationship with age, gender, refraction and (BCVA) were analyzed.Results Older age was significantly associated with CNV and macular atrophy (OR=1.034,CI=1.019-1.049,P<0.001;OR=1.054,CI=1.031-1.076,P<0.001; respectively);younger age was associated with hemorrhage with LC (OR=0.906,CI=0.876-0.937,P<0.001). Higher myopic refractive error was associated with macular atrophy (OR=0.762,CI=0.705-0.824,P<0.001), whereas lower myopic refractive error was associated with CNV and macular hole(OR=1.233,CI=1.136-1.338,P<0.001;OR=1.554,CI=1.185-2.038,P<0.001; respectively). A worse visual acuity was associated with CNV (OR=1.835,CI=1.180 -2.854,P=0.007), while better visual acuity was associated with LC (OR=0.506,CI=0.328 - 0.782,P=0.002). There was no gender difference in distribution of high myopic maculopathy types. Conclusions Pathologic myopic maculopathy can be divided into six types. With increasing age, the incidence rates of CNV and macular atrophy increases, hemorrhage with LC but decreases. With the rise of myopic refractive, the incidence rates of CNV and macular hole decreases, macular atrophy but increases.

  • Macular morphological changes of choroidal melanoma with optical coherence tomography after plaque radiotherapy

    Objective To observe the macular morphological changes of choroidal melanoma with coherence tomography (OCT) after plaque radiotherapy (PRT). Methods A total of 48 patients (48 eyes) with choroidal melanoma who underwent125I PRT were enrolled in this study. All the patients were examined documenting OCT to get the image of macula. The macula of all the patients was not involved. The median visual acuity was 0.4plusmn;0.2, which ranged from 0.02 to 1.0. There were 18 eyes (37.5%) with retinal detachment, 12 eyes (25.0%) with retinal pigment epithelium (RPE) changes, seven eyes (14.6%) with macular edema, epimacular membrane, detachment combined with edema, exudation and RPE changes, 11 eyes (22.9%) with normal macular structure. The median follow-up time was (10.4plusmn;5.9) months, which ranged from one to 24 months. The tumor control situation and visual acuity were observed in follow-up period. The same equipment and methods of OCT were used to return visit in follow-up period. The macular morphological changes at the final visit and its relationship with PRT and visual acuity were contrastively analyzed. Results All the patients had good control of tumor. The vision acuity improved in two eyes (4.2%), unchanged in 10 eyes (20.8%), and decreased in 36 eyes (75.0%). The differences of the visual acuity was statistically significant between before and after treatment (Z=-3.778,P<0.05). There were 13 eyes (27.1%) with retinal detachment; nine eyes (18.8%) with RPE changes; 17 eyes (35.4%) with macular edema, detachment combined with edema, exudation and RPE changes; six eyes (12.5%) with proliferation, atrophy, detachment combined with edema, exudation and epimacular membrane;three eyes (6.3%) with normal macular structure. There were 15 patients (31.3%) with two or more abnormal macular morphology after PRT. Conclusions Retinal detachment, RPE changes, macular edema and exudation are common abnormal macular morphology after PRT. The incidence rate of abnormal macular morphology is increased. There are 31.3% patients with two or more abnormal macular morphology.

  • Application of AccuMap multifocal visual evoked potential on visual function in macular diseases

      Objective To evaluate the value of AccuMap multifocal visual evoked potential (mfVEP) objective perimetry in measuring visual function of macular diseases. Methods Forty-six eyes (37 patients) with macular diseases were studied. The mean mfVEP amplitudes of central visual field within radius 2.0deg;, 5.5deg;, 10.0deg;, 15.5deg; and 24.0deg;were measured by AccuMap mfVEP objective perimetry. Automatically calculated AccuMap severity index (ASI) represented the visual field defects. Meanwhile, the mean sensitivity (MS) of central visual field within radius 2.0deg;, 4.0deg;, 6.0deg;, 8.0deg; and 10.0deg; and mean defect (MD) of central visual field within radius 10.0deg; were assessed by MP-1 microperimeter. The correlations among those parameters including logMAR visual acuity, mean mfVEP amplitudes, MS, ASI and MD were analyzed. Results Among 46 affected eyes, AccuMap mfVEP objective perimetry showed abnormal ASI with visual field defects in 43 eyes (93.45%), normal ASI without visual field defects in three eyes (6.55%). The mean mfVEP amplitudes value of the central visual field within radius of 2.0deg;, 5.5deg;, 10.0deg;, 15.5deg; and 24.0deg; were (120.78plusmn;52.91), (134.31plusmn;54.67), (149.99plusmn;52.42), (156.69plusmn;48.30), (157.54plusmn;45.07) nV respectively. The mean ASI value was 74.26plusmn;52.43. MP1 showed that the MS values of central visual field within radius 2.0deg;, 4.0deg;, 6.0deg;, 8.0deg; and 10.0deg; were (10.97plusmn;5.91), (12.71plusmn;5.13), (13.71plusmn;4.53), (14.23plusmn;4.18), (14.48plusmn;3.86) dB respectively. The MD value of central visual field within radius 10deg; was 4.49plusmn;3.07. There was a significant correlation among ASI, the mean mfVEP amplitudes of central visual field with every radius and logMAR best corrected visual acuity (P<0.05). The mean mfVEP amplitudes of central visual field within radius 2.0deg;, 5.5deg;, 10.0deg; were positively correlated with the MS of central visual field within radius 2.0deg;, 6.0deg;, 10.0deg; (r=0.477, 0.432, 0.433;P=0.001, 0.003, 0.003). Furthermore, ASI was also correlated with MD of central visual field within radius 10.0deg;(r=0.729, P=0.000). Conclusions AccuMap mfVEP objective perimetry can reflect the changes of macular function objectively and can be used to assess the visual function of macular diseases.

  • Macular function of pathologic myopic retina evaluated by microperimetry

      Objective To observe the microperimetry performance of macular function in pathologic myopia patients. Methods The clinical data of 90 patients (142 eyes) with pathologic myopia were retrospectively analyzed. All patients were asked in details about history, and take examinations of best corrected visual acuity (BCVA), refractive dioptre, eye axis, fluorescent fundus angiography (FFA), indirect ophthalmoscopy and optical coherence tomography (OCT). According to the test results, patients were divided into non-pathological macular group (20 patients, 24 eyes) and pathological macular group (70 patients, 118 eyes). Retinal imaging and macular microperimetry were measure by MP-1 Microperimeter.The mean retinal sensitivities (MS) and fixation stability in the central 10deg;, fixation rate and fixation position in the central 2deg; and 4deg;were determined.Results The MS of pathological and non-pathological macular group were(16.39plusmn;2.12), (10.80plusmn;4.53) dB respectively, the difference was statistically significant(F=15.044,t=-9.314;P=0.000). Among 24 eyes of non-pathological macular group, fixation was stable in 19 eyes (79.17%), relative unstable in five eyes (20.83%); among 118 eyes of pathological macular group, fixation was stable in 45 eyes (38.14%), relative unstable in 52 eyes (44.07%), unstable in 21 eyes (17.79%), the difference was statistically significant(chi;2=13.56, P=0.000). The differences of 2 deg;and 4 deg;fixation rate between those two groups are statistically significant (F=5.773, 13.230; t=-4.110,-5.465;P=0.000) . Among 24 eyes of non-pathological macular group, center fixation occurred in 23 eyes (95.83%), weak center fixation occurred in one eye (4.17%); among 118 eyes of pathological macular group, fixation center occurred in 81 eyes (68.64%), weak center fixation occurred in 16 eyes (13.56%),eccentric fixation occurred in 21 eyes (17.80%), the difference was statistically significant (F=9.618,t=-5.773;P=0.000).Conclusion Pathological myopia patients with pathological macular changes have decreased retinal sensitivity, decreased fixation stability and eccentric fixation points.

  • Macular thickness measurement of six-year-old children and analysis of its relevant factors

      Objective  To observe the distribution of macular thickness and its relationship with axial length, gender, refraction status and race in six-year-old children. Methods 1543 six-year-old children from 34 schools in Sydney were randomly selected. The thickness of center macula(CM), inner macula(IM)and outer macula(OM)were measured by optical coherence tomography(OCT), the differences of macular thickness between different genders and races were comparatively analyzed. The axial length and refraction status of all children were measured by IOL Master and autorefractor RK-F1 respectively. Results The CM, IM and OM thickness of all those six-year-old children were normally distributed,on average they were 193.6, 264.3 and 236.9 mu;m respectively,the differences were statistically significant(t=0.40,0.08,0.20;P<0.05). The CM,IM and OM average thickness was 194.2,264.9,229.0 mu;m for boys and 189.3,262.5,231.5 mu;m for girls respectively. CM and IM thickness of boys were thicker than those in girls(t=0.15,0.11;P<0.05). The CM,IM and OM average thickness was 196.0,265.2,237.5 mu;m for White kids,186.0,262.3,236.5 mu;m for East Asia kids and 19.4,263.7,233.8 mu;m for Central East kids respectively. The CM and IM thickness of White and Central East children were significantly thicker than East Asia children (t=0.12,0.15;Plt;0.05). There was no difference between these three races in OM thickness(t=0.16,P=0.6). The IM and OM were thinner with longer axial length (Plt;0.05). The CM,IM and OM thickness was thicker with higher hyperopic degrees (Plt;0.05). Conclusions Macular thickness was normally distributed in six-year-old children and was significantly affected by gender, race, axial length and refraction status.

  • Analysis of the images of optical cohference tomography in the patients with macular hole caused by closed ocular trauma

    Objective To observe the features of the images of optical coherence tomograpy (OCT) in patients with traumatic macular hole (TMH), and detect the clinical significance of OCT. Methods Consecutive 74 patients (74 eyes) diagnosed with TMH by examinations of visual acuity, slit lamp, and direct or indirect ophthalmoscopy underwent optical coherence tomography (OCT). The analysis software of OCT was used to make the quantitative measurements of TMH. And the TMH were classified according to the morphological characteristics of the images of OCT. 50deg;color fundus photography was performed on the patients after OCT. The relationship of TMH with the average visual acuity, disease duration, average neuroepithelial thickness on the margin of hole, and the base diameter and the apex diameter of macular hole were retrospectively analyzed. Results The characterisctics of the images of 74 cases (74 eyes) of TMH were classified into 5 types: macular holes with symmetric edema of the neurosensory retina at the margin in 27 eyes (36.5%), macular holes with asymmetric edema of the neurosensory retina at the margin in 12 eyes (16.2%), macular hole with full-thickness defect of neurosensory retina without edema or detachment at the margin in 14 eyes (18.9%), macular hole with localized detachment of the neurosensory retina at the margin without edema in 17 eyes (23.0%), and macular hole with thinning neurosensory retina in 4 eyes ( 5.4%). There was no significant difference of visual acuity among different types of TMH (F=1.574, P=0.191). The visual acuity was positively related with the marginal retinal thickness (r=0.342, P=0.003), but not related to age, diameter of macular hole or the disease duration (r value was from -0.022 to -0.134,P value was from 0.863 to 0.261). The disease duration of Type IV TMH was shorter than that of other TMH types. In the patients with the disease duration over 90 days, Type I TMH was predominant. The average retinal thicknesses at the margin of the hole showed significant differences among different TMH types (F=13.921, P=0.000). Conclusions TMH could be divided into 5 types according to the characteristics of images of OCT; the clinical characteristics of different types of TMH varies. 

  • The diameters of macular hole and destroyed boundary line between inner and outer segment of the photoreceptors and the correlation with the visual acuity in patients with idiopathic macular hole

    Objective To observe the diameters of macular hole and destroyed boundary line between inner and outer segment (IS/OS) of the photoreceptors and the correlation with the visual acuity in patients with idiopathic macular hole(IMH). Methods The clinical data of 39 eyes (37 patients) with IMH were retrospective analyzed. All the patients had undergone the examinations of visual acuity (Snellen chart), intra-acular pressure, ocular fundus (indirect ophthalmoscope), and Fourier-domain optical coherence tomography (FD-OCT) whose speed was 27 000A scan/s, area was 6.0 mmtimes;6.0 mm, and the mode was 512times;128. The diametres of macular hole and destroyed IS/OS, and the correlations with visual acuity were detected. Results The mean logMAR was 0.99plusmn;0.44 (ranged from 0.15 to 2.00),the mean diameter of macular holes was (942.0plusmn;348.4) mu;m(ranged from 171 to 1491 mu;m), and the mean diameter of IS/OS disruption was (1870.3plusmn;673.2) mu;m(range from 463 to 3176 mu;m). There was a significant correlation among the diameter of the macular hole, the diameter of the IS/OS disruption, and logMAR in IMH (P=0.038, 0.002, 0.000). In eyes with closed macular hole after surgery, the logMAR and the diameter of the IS/OS disruption had a significant decrease. Conclusion Using FD-OCT the photoreceptor changes can be visualized in vivo. The abnormality in the ISOS boundary line appears to be involved for a much larger area beyond the macular hole itself, and persists there with small size even after the macular hole closure surgery.

  • Characteristics of idiopathic epiretinal membrane by new Fourier-domain optical coherence tomography

    Objective To observe the Fourierdomain optical coherence tomography(FD-OCT)characteristic of idiopathic epiretinal membrane (ERM) and its correlation with the patients visual acuity. Methods The 116 eyes (112 patients) with idiopathic epiretinal membrane were included in this study. All the patients had undergone examination of visual acuity (LogMAR chart), ocular fundus (direct ophthalmoscope and fundus pre-set lens), and FD-OCT (Zeiss HD-OCT) with the speed of 27 000 A scan/s,area of 6.0 mmtimes;6.0 mm, and mode of 512times;128. The central fovea thickness (CFT), volume (V), average thickness (AT) were collected from the OCT readings, and the fovea thickness (FT) was measured manually. Data were analyzed using SPSS 16.0. Results The traction on retina caused by ERM can be divided into 3 types according to the OCT findings: no traction (9.48%), tangential traction (84.48%) and tangential traction with anteriorposterior traction (6.04%). A total of 97 eyes (83.62%) had retinal edema (diffuse or cystoid) and the edema located at the outer nuclear layer(ONL), outer plexiform layer(OPL)and inner nuclear layer (INL) . A total of 14 eyes (12.07%) had retinal nerve fiber layer (RNFL) schisis and 27 eyes (23.28%) had inner segment/outer segment junction (IS/OS) impairment. Statistic analysis revealed that BCVA was not related to the age, gender, types of traction, presence of IS/OS damage or RNFL schisis (Pgt;0.05), but was related with CFT(Plt;0.05). Conclusions Idiopathic ERM can exert different types of traction on the macular, and cause different types of retinal lesions. OCT is a useful tool to measure these lesions. CFT reading is closely related to patients' visual acuity.

  • Correlation between the central macular thickness and the visual function in patients with macular edema

    Objective To determine the correlation between central macular thickness (CMT) and the visual function in patients iwht macular edema (ME). Methods The clinical data of 42 eyes of 40 patients with ME which were examined by optical coherence tomography (OCT) and microperimetry (MP-1) were retrospectively analyzed. In 40 patients (42 eyes), diabetic ME (DME) was in 27 eyes,branch retinal vein occlusion was in 11eyes, and central retinal vein occlusion was in 4 eyes. All of the eyes had undergone OCT,MP-1 and best-corrected visual acuity (BCVA) test. Central macular thickness (CMT) was measured by fast macular scans using OCT. Retinal sensitivity (MS) and fixation patterns were evaluated by Mp-1. The position was chosen :2 disc diameters (DD) temporal to the disc and one third of a DD inferior to the centre of the disc. Results The correlation between CMT and BCVA is not significant (r=-0.429,P=0.069) as well as the correlation between CMT and MS (r=-0.433,P=0.058). The difference of CMT between the unstable and stable group was significant (F=3.262, P=0.039). The difference of CMT between the central fixation group and preferred retinal locus (PRL) group was significant (F=3.173, P=0.044). Conclusions BCVA and MS have no significant correlation with CMT. When CMT increases, the fixation stability decreases, fixation location.changes, and PRL occurs.
